Below you will find common forms that will need to be completed in order for us to have the information necessary to process your pay, tax filings and/or benefits.  Not all forms are required for all positions, so please note in the New Hire Checklists provided for each common position what forms you will be responsible for filling out.

District Policies.

Direct Deposit Form
Direct deposit is mandatory. Please complete the form and include a voided check or deposit ticket.

Federal W-4

I-9 Employment Eligibility Verification
Please complete page 1 of the Form. (This form can be completed online and printed out) Instructions for completing the form can be accessed by clicking the Instructions button at the top of page 1. Please also note that you will be required to submit acceptable documentation. A list of acceptable documentation can be found on page 4.

Employee 1095-C Forms
1095-C Forms will be available electronically; opt-out option for printed copy. RV employee 2024 and 10-95-C forms will be available for viewing via request.

Benefits of choosing an electric copy:
- It will be ready earlier than print copies.
- It is a safe method of delivery considering the documents contain sensitive data such as social security numbers.
- It can be accessed from your email.
To receive your 10-95-C electronically instead of a paper copy, please complete the opt-out form HERE.
Employees have until March 15th to change their preference to receiving their forms electronically.
